FTJ – New Therapy Provider and Program

Franke Tobey Jones is pleased to announce our new rehabilitation and wellness program called AgeSTRONG by Consonus Rehab.  We are committed to ensuring residents have access to the best services to achieve and maintain their highest levels of health, wellness, and fitness. AgeSTRONG is taking the place of Infinity Rehab as our therapy provider. This new program will deliver physical, occupational and speech therapy services as well as offer individual personal training programs, provide health education and collaborate with our wellness department for future enhancements. All residents in Independent Living, Assisted Living, Memory Care and Skilled Nursing on campus will be able to participate in services.

Consonus is a leader in the pharmacy and rehab therapy industry, offering proven clinical programs to support improved quality of life and functional outcomes. AgeSTRONG by Consonus Rehab has a dedicated team of therapists and fitness specialists, and we are so pleased that our current Physical Therapists will remain at Franke Tobey Jones as part of the new AgeSTRONG team along with additional team members to support the program.

Initially, the main areas of service provided by AgeSTRONG at FJT will be:

• Expanding Outpatient Physical, Occupational and Speech Therapy services
• Personal Fitness Training
• Health Talks
• Fitness screenings to capture and analyze data to prevent problems before they affect you mentally or physically.

AgeSTRONG is based on a model of care that a Stanford Health Care study found can result in lower mortality rates; faster time to achieving goals and regaining mobility; and greater patient experience.

Since launching a year ago, AgeSTRONG has expanded to five states (Oregon, Washington, Arizona, California and Texas) and has doubled its clients in participating senior facilities. More than 90 percent of those clients said they would recommend AgeSTRONG to family and friends.
